Our 45 min examinations are much longer than the average 17 minutes, ensuring good clinical care.
This comprehensive consultation includes the following:
- Pretesting auto refraction and vertometry of previous habitual spectacles
- Cardiovascular screening tests: blood pressure measurement (patients over the age of 38)
- Measurement of intra-ocular pressure which make part of the glaucoma screening and which is asked for by various medical schemes.
- We perform other tests like retinoscopy, slitlamp examination , vision acuity without spectacles and vision with the new tested refraction,(non-cycloplegic and cycloplegic with pupil dilation where necessary) as well as binocular evaluations.

Our Passion: Willem Pieterse of Pieterse Optometrists specialises in visual solutions for problems associated with keratoconus, post corneal surgery complications and rehabilitation of irregular corneal cases. Caring for an exceptionally large contact lens patient base, advanced contact lens fitting for irregular corneal cases has become a passion and interest in his profession. In addition, he specialises in the fitting of Orthokeratology lenses for progressive myopes.

What is Keratoconus:
Keretoconus is an eye condition where the normally round dome shaped cornea progressively thins, causing a cone-like bulge to develop. This results in significant visual challenges over time. Symptoms as listed usually first appear in the late teens. Keratoconus may progress for 15-20 years before the progressive changes stabilize; often vision is affected badly over these years leading to a need for corneal transplant surgery. This is a surgical procedure that replaces the keratoconic cornea with healthy tissue.
New technology has resulted in a new contact lens called a “scleral lens”’ which gives great comfort. If keratoconus is diagnosed early, a new surgical procedure called corneal cross-linking can be done to stop the progression of keratoconus completely.
What we do: Modern contact lenses have placed RGP lens fitting into a different league.
In our practice we have invested extensively in the latest technology including Topography, ensuring exemplary clinical standards and supporting accurate examinations and contact lens fitting.
It is essential that keratoconus is identified at a young age and since keratoconus has a large genetic component, all family members should have a Topography screening done to identify corneal changes.
What is different about scleral lenses:
Scleral contact lens fitting is a new option specifically developed for keratoconus in order to assist in correcting patient’s vision after corneal transplant surgery, radial keratectomy (RK) or post cross-linking/keraring treatments. It is often the only option when all else have failed and the results with these lenses are often the best.
The most significant difference is the exceptional comfort (similar to soft contact lenses) and improved vision compared to previous hard lenses. Scleral lenses create a unique environment of constant liquid on the eye. This results in the absence of friction and still provides adequate oxygen.
ORTHO K TREATMENT PLAN
What is OrthoK?
OrthoK (short for Orthokeratology) is the only non-surgical procedure that allows visual clarity without the use of correcting lenses during the day. It is a revolutionary therapy in South Africa which involves the use of specially crafted contact lenses, worn overnight, to gently reshape your cornea whilst sleeping.
Orthokeratology is completely different to the general contact lens available in South Africa and it should be clear that this is a FDA approved contact lens for night time wear.
The contact lens retainer is worn for 6-8 hours at night during which time the lens gently reshapes the first layer of the cornea called the epithelium. Upon awakening, the corneas are the correct shape to accurately focus light, enabling visual clarity without the hassle of contact lenses or glasses during the day!
The retainer lenses are comfortable, easy to care for, and offer relief from the problems sometimes associated with full time contact lens wear like dryness, allergies, and foreign bodies getting on or under the lenses.
How does OrthoK work?
OrthoK works by gently remoulding the front surface/window of the eye called the cornea.
It does this by using the tears underneath a specially designed retainer lens. The cornea has an anatomical structure of 5 different layers and the contact lens retainer causes a temporary change in the first layer of the cornea called the epithelium.
The tear film under the lens actually does all of the work, causing a slight microscopic fluid pressure in the centre and a slight pulling effect in the periphery of the cornea. The lens itself creates a shape like a dental retainer; the lens is fitted slightly closer to the central/middle cornea and further away with a larger gap between the retainer and the eye in the peripheral cornea.
The first layer of cells is altered by approximately 20 microns causing the effect needed to see clearly. The lens itself does not physically press on the eye in any way. (OrthoK video available at Youtube.com).

Diabetic Eye Disease:
South Africa is one of the countries with the highest undiagnosed diabetes, an illness with detrimental effects on vision which can, if not diagnosed and addressed timely, can lead to permanent loss of vision. Diabetic retinopathy is mostly unnoticed by the patient until it has affected the eye to a great extend and therefore a dilated retinal examination is crucial for patients suffering from diabetics. Early detection is the key!
The 3 most common eye complications of diabetes:
- Cataracts – Clouding of the lens of the eye
- Diabetic retinopathy- Damage to the blood vessels in the retina – Most commonly seen
- Glaucoma – Complication of the optic nerve causing loss of peripheral vision
Diabetic retinopathy means that the retina layer which is situated very deep in the eye starts to bleed and this is one of the serious complications of diabetes which can eventually leads to blindness if untreated.
At least 50 % of patients develop some kind of diabetic retinopathy at 5 years or more after the onset of their diabetes. 3 out of 10 people with type 2 diabetes have already developed complications by the time they are diagnosed. 95 % of severe vision loss from Diabetic retinopathy can be prevented by early detection, timely treatment and appropriate follow up.
It is therefore essential that people with diabetes are screened yearly to pro-actively identify any early signs of diabetic retinopathy. New treatment options are highly affective to avoid vision loss, especially if implemented at an early stage.

MYOPIC PREVENTION & REFRACTIVE TREATMENT
The fourth group of patients that we specialise in, is pediatric patients of age between 8 and 15 years needing assistance with myopic progression control. High myopic adult patients also have a need to be spectacle or contact lens free during the day having similar myopic problems to patients that want refractive correction surgery but without the risk of eye surgery. The patients will come mainly from referring professionals such as general practitioners, eye specialists, optometrists and other medical professionals, as well as from direct marketing from informative talks and presentations.
We specifically focus on pediatricpatients, by means of special treatment offers to this age group and also offer services at schools.
Pediatricpatients needing myopic progressing control are fitted with ortho-keratology RGP contact lenses. The lenses have shown very good results in the USA and in Europe, showing a decrease of myopic progression of 50-60%. It is better for younger patients to be spectacle or contact lens free during the day and give the parents the control about eye care management during the night at home.
The adult patients will also benefit by ortho-keratology lenses making these patients’ spectacles and contact lens free during the day. Individuals also wear the contact lenses at night and have the benefit and freedom of being spectacles free during the daytime. Patients get similar results to refractive surgery knowing that is totally reversible and a non-surgical procedure. The Ortho-K contact lens patients also benefit from the lens design that help see clearly in the day and assist to avoid reading spectacles after the age of 40 years. The advantages of these permanent lenses are that the cost is significantly 60% less compared to eye surgery without the risk.
